Patients are seen by appointment only. Appointment schedules vary according to provider availability. We are happy to reserve a time in your provider's schedule, however, in consideration for others,
we do request at least a 24-hour notice prior to cancellation of any appointment. We do understand that there are circumstances that may prevent you from keeping
your appointment. If you provide us with as much notice as possible, we may be able to contact another patient who needs an appointment. Appointments fill quickly,
and cancelling with less than 24-hours notice does not allow enough time to schedule another patient in need of treatment. Therefore, a cancellation or no-show fee
will apply if our office is not notified that you will be unable to make your appointment.
At the time of your appointment, you are required to present a copy of your drivers license and all insurance cards. Failure to do so will make you responsible for paying for your visit in full.
Any patient financial responsibilities are due at the time of service. For your convenience we accept cash, checks and credit cards.
At Lake Area Psychiatry, we strive to ensure a seamless process for prescription requests and refills. Please follow these guidelines to help us serve you efficiently:
To avoid interruptions in your medication, please submit your request well in advance. If you have any urgent concerns about your medication, please contact our office. Thank you for allowing us to be a part of your care!
