Lake Area Psychiatry office hours are:
Monday – Thursday | 8:00 a.m. – 5:30 p.m.
Friday | 8:00 a.m. – 12:00 p.m.
Patients are seen by appointment only.
Appointment schedules vary according to the provider of service. We are happy to reserve a time in your provider’s schedule, however, in consideration for others, we do request at least a 24-hour notice prior to cancellation of any appointment. We do understand that there are circumstances that may prevent you from keeping your appointment. If you provide us with as much notice as possible, we may be able to contact another patient who needs an appointment. Appointments fill quickly, and cancelling with less than 24-hours notice does not allow enough time to schedule another patient in need of treatment. Therefore, a cancellation or no-show fee will apply if our office is not notified that you will be unable to make your appointment.

In an effort to better serve our patients, we have implemented an automated voice attendant to assist with your telephone requests. We make every effort to answer your telephone call promptly, however, when our staff is busy assisting patients, your call will be directed to the automated attendant, at which time you will select the option that best meets your need:
Appointment Scheduling, Rescheduling, or Appointment Cancellations, Select Option 1
Prescription Refill Requests and Questions Regarding Your Medication. Select Option 2
Insurance / Monthly Billing Statment Questions, Select Option 3
Medical Record Requests, Select Option 4
BILLING AND FEES
Your patient FINANCIAL RESPONSIBILITY is due at the time of service.
For your convenience we accept cash, checks and credit cards.
INFORMATION REQUIRED
On or before your first visit, you will be required to complete patient information forms. This information is necessary to assist in filing your insurance or handling your particular situation. A consent to release information and authorization for your insurance company to pay our facility for services rendered to you are also included. All information you provide us is strickly confidential.
PRESCRIPTION AND REFILL REQUESTS
Prescriptions and requests for refills are handled during the regular business hours.
To request a prescription or refill,
please call the office at 337-478-9331.

Leave the following information on the voice attendant:
- Your Name
- Name of Medication
- Pharmacy Name and Telephone Number
- Prescription Number
- Your Telephone Number or Number where you may be reached
- Date of your next appointment
Your medication will be refilled as soon as your doctor authorizes the refill. If you have questions about your medication, please leave your number and message and your call will be returned as soon as possible.
Please allow 5-business days to process your request for medication refills.
